The nurses there were wonderful humans who helped a tired, sometimes struggling new doc through difficult shifts. Cheers to the #ICUNurses, we lost a great one today.

- Biotin was recommended for one of my kidney transplant patients for hair loss associated with tacrolimus. Oral doses may interfere with PTH assays.

- I think this was the expected finding based on the original BICAR-ICU trial. In that one, 52% of the control group received RRT (and therefore bicarb) with a mean of 7 hours from enrollment. With poor separation of treatment arms, it’s hard to attribute the mortality benefit to the intervention.
- The BICAR-ICU-2 trial just dropped! RCT involving IV bicarb in metabolic acidosis among patients with mod-severe AKI Bicarb didn't affect mortality but it reduced the need for dialysis & risk of bacteremia VERY similar conclusions to BICAR-ICU-1 🧵 #1/ jamanetwork.com/journals/jam... #EMIMCC
