View full threadOn Segun’s point re refuting the decision - I find, again it’s about the challenging nature of a conversation where you have to admit therapeutic failure. Is it the role of the deciding clinician to speak to the family?
- Would I expect the oncologist to say there are no chemo options? Or is it the preserve of the referring respiratory doc post MDT? Should the cardiologist tell the family that the patient’s MIRACLE2 score is 7 therefore no PCI right now? Or should ICU in person communicate the ceilings of care?

- One aspect of this is the referring team being uncomfortable in the role of breaking bad news. Saying to a patient that further therapies and escalation are futile is often shied away from. Far “easier” to have another clinician say this.
