They are barred from negotiating directly. But they approve the part D and MA plans that have PBMs that over charge, discharge , underpay pharmacies and more.
They also approve the ridiculous coinsurance percentages.

What I'm getting at is that there are so many money-motivated middlemen, like insurers and PBMs, even in government funded Healthcare, and this is no mistake. It's outsourcing government business to private entities that value profit over patients. No other sane country is doing this.
