- I have created a backup of every incident case study from The chemical safety board. Next step will be extracting all the recommendations to the master database. You can browse for them www.inquestsandinquiries.com/inquiries by just selecting the CSB tag and pressing apply. Phew!
- This weeks update - the Brook House inquiry www.inquestsandinquiries.com/projects/bro... . The page includes a new AI podcast of the report and you can look at all the recommendations by searching the report name here www.inquestsandinquiries.com/recommendati...
- Sorry for the long wait episode 1 of the second series of the inquests and inquiries podcase is the Vale of Leven inquiry and Scottish government response, following the C Difficile outbreak at the hospital. www.podbean.com/eas/pb-qnrya...
- and finally (because coding waits for no time wasting distractions) the relatively mainstream (compared to the others) Peking Duck. Showing Ozzies can still bang out a catchy song and weird video
- turning the weird up a little more... the rubber bandits (great lads) and some amazing moves
- which leads me to the delight from talking heads, the very underplayed 'and she was...'
- and this beautiful version of wonderful life combines German reggae, explosions and puppets. you are welcome...
- I’ve a week off from my main job so hope to make some progress on the inquiries website update. The two big changes coming will be a bit less flash and a bit more simplicity on the landing page and the recommendations page is just getting a full refresh. I’m trying to improve the search too
- Packed day, but nice highlight talking to a NILO CPD day about Inquiries (with a little AI razzmatazz). I need to stop thinking I can fit a million thoughts in to 40 mins! Always happy to chat to people about this and introduce you to what some of the tools can do, and how to get the most from them.
- Amazing cause - and it involves respirators and a nails challenge. Please dig in and dig deep…
- I love a good video - and now I have created the perfect clip from a video for me to use a gif when I read some utter borscht quasi science on social media. Brace yourselves because....

- Working on a big paper to drop soon on risk registers. I have found a gem of a phrase in a paper that talks about “schrodinger’s ostrich”. Want to know more? Comment below!
- This weeks episode, the Redfern inquiry and the human tissue scandal. buff.ly/3CUV1gI . Deeply tragic about how families were treated and very similar to the challenges of the families whose loved ones were tested due to coal mining.
- Well that was a week... so definitely need some soothing tunes!!! buff.ly/3X0Iteq This is so catchy and great video. Good booster for the gym too!

- Exploring the government's response to the UK Covid-19 inquiry. What does it mean for us? I promised an analysis - and here it is. Lots of links, sources and even a bit of AI. Read my insights here: buff.ly/437eJ3B #Covid19 #GovernmentResponse #HealthTrends
- Just to warm you all up, I have gone through the Government response to the Covid inquiry recommendations for module 1. Full blog post and podcast tomorrow (as well as fully updated COVID page on the site. But as a teaser.. its a disappointment.
- A little more work on the site. Tonight, I've added a lot to the Aberfan page (podcasts, videos etc.). buff.ly/3WLvsFo I find Aberfan a really tough inquiry to read. A horrendous example of failing to protect people from known risks.
- For those speculating about air accidents. Don’t. Wait for a proper investigation, you aren’t helping and you are making it worse. Todays daily highlight: the tragic Marchioness disaster
- As the site expands, each day I will showcase an inquiry. Today the Vale of Leven inquiry buff.ly/3CxqIMG which looked at harm from poor infection control measures
- As the site expands, each day I will showcase an inquiry. Today the foot and mouth inquiry -
- @lucygobag.bsky.social good heavens your follows are huge already!!!
- a labour of love (sorry for people who I have been errant in talking to - been buried!), and it still has edges to knock off. Some import errors in the text on the data sets added random symbols. the shape and the structure for collaboration so I'm not doing it all ).
- oh oh oh... and a feature in the inquiries pages that will come soon. As I review each, I tag it for common themes (leadership, candour etc.). Next weekend I will hopefully turn that on so you can spot common recommendations and thematic problems.
- Finally (I desperately need sleep!) you can join the site (no charge), with an app for your phone too. Nothing in the group yet, but it has a file share system that will make it a lot easier for me to safely share all the files with you and vice versa.
- A lot has gone in to upgrading the way to search inquiries. now you can select a theme then click on an inquiry. The inquiry pages (for now only Health is complete) have a LOT of info, downloads, videos, podcasts etc.
- Major update to the website. There is a lot still under construction, as I have moved to a dynamic page with database. First of... you wanted a searchable database of recommendations. Meet version 1:
- Great book. Notably, you shouldn’t really be looking to regulators to cause growth. They can protect it, keep regulations under review but they are there to prevent harm. I do get why HMG asked this but this is why they should have an Inspector General not 100s of bodies.
- If you wondered how well the government manage the knowledge gleaned and placed on inquiry websites. The answer is 'not great, really not great'. Reliant on the internet archive, which isn't ideal.I found it catalogued the wrong site for the standards in public life inquiry. So that's lovely.
- Ok... probably only got time for one podcast deep dive this weekend. Any requests for an inquiry or theme you would like summarised. Following the Southall podcast, I have had one asking to analyse 3 of the major Rail inquiries?

- I have a lot to do, and some exciting new features, for the website but in the meantime here is the next episode of the podcast looking at inquiries. This week, the Southall Rail Accident.
- On Mon, I mentioned I don't like risk matrices. For those that asked "why?" and "what can I do to improve them?" here is a little taster video about the issues and some answers! buff.ly/4g87B9I #risk
- It’s a day off, which means time to do my other job! Just getting ready for a morning on risk analysis for the buff.ly/3WejkfK Masters students and the cranfield.ac.uk/courses/short/defen…

- Yet the same is repeated - notably in the Post Office inquiry where the claims of ignorance by senior management have a chilling echo of the NCB HQ’s own protests.
- Just one, Aberfan, is heartbreaking. 116 children killed in a slide of mining waste. The inquiry found “not wickedness, but ignorance, ineptitude and failure in communications” in 1966.
- Catching up on my reading and plans for my inquiries website, and I am ever more certain I need to make this the direction of the rest of my career. Each inquiry I read, often at the expense of many deaths, is too precious for the casual way they are managed.
