- This week I had a “rapid car clean-up” experience – unfortunately I was one of the many collisions on the ice. Crashed car admin is the only thing worse than having to quickly empty all the pockets and car cupboards before the recovery arrives. 🚙 fortunately no one was hurt #DriveToArive #nurse
- I have a question How is decreasing the payment allowance for travel supporting the Long Term Plan - hospital to community? “car mileage rates will drop from 59p to 56p per mile for journeys up to 3,500 miles annually” Many community nurses use their own vehicles to visit patients. Is this ok?!
- Gentle reminder - if you have to speak to someone in the NHS over the next couple of weeks, know this: The person you speak with is just as frustrated as you - they don’t need to be shouted at, told they don’t care, or even sworn at. I’ve worked all weekend - I’ve done my best & so have the team.

- There’s a need to consider how services are designed and modelled, in order to be able to deliver safe effective care in the community. With the ‘promise’ of shifting care from hospital to community, let’s involve those with the insight and imagination to make changes that will be sustainable

- District nursing is in a dire state, and the latest report from @nuffieldtrust.org.uk shows a worse than feared collapse in the workforce. Westminster government’s ambitions to shift care from hospital to community will fail unless the district nursing workforce is rebuilt. ⬇️

- This is proactive preventive use of services. Maybe open referrals from District Nurse / UCR teams who will be able to name the people in their communities who would benefit from CGA type review?
- Currently: 1. Reaching out to GP/AHP 2. Triaging general medical referrals so we see older ppl. 3. Extending offer to see appropriate ppl referred to other specialty eg colorectal/cardiology. 4. Great conversation with ENT colleague today. Plans to divert older people referred with 'instability'.2/2
- I’ve been thinking a lot about how frontline nursing has changed — not just in what we do, but in how we lead. We’re no longer just implementers. We’re assessors, coordinators, advocates — and we need tools that actually support that. 1/ 🧵
- Digital systems should fit how we work, not the other way around. Real-time collaboration, less duplication, and fewer delays. That’s how we keep care safe and responsive. 2/ 🧵
